Registration certificate issued by the Ministry of Trade, Industry and Investment Promotion.
Certificate of affiliation to the Oman Chamber of Commerce and Industry.
employment contracts under which radioactive sources will be used.
Certificate of origin and leakage tests for radiation sources.
Technical guide (catalogues) for radiation devices.
A copy of the radioactive sources storage facility license issued by the Authority.
A radiation protection program that is appropriate to the nature of the practice and includes the following:
A list of names of workers in the radiation field, naming the radiation protection supervisor.
CVs of workers in the field of radiation with training certificates in the field of radiation protection.
Documents for the participation of all workers working on radiation sources in the facility in personal radiation monitoring.
A description explaining the mechanism of spatial radiological scanning, along with documents and certificates of radiological scanning devices.
A list of radiation protection devices and equipment used in the facility.
A radiation emergency plan approved by the Civil Defense and Ambulance Authority that is consistent with practice.
