• A description of the measurement system for personal radiation monitoring meters and a description of the specifications of the devices used (personal radiation monitoring meters and personal radiation monitoring meters’ reader).
• A description of the services to be provided to subscribers.
• A description of the method for calculating radiation doses from personal radiation monitoring standards.
• A description of the procedures that will be followed in the event that personal radiation monitoring standards are damaged or not returned.
• Description of the quality control program for the measurement process.

• A list of the names of qualified lecturers for the purposes of providing these services.
• Qualifications and experience of the lecturers (a PhD/Master of Science, Health Sciences or Engineering with 5 years of experience or a Bachelor of Science, Health Sciences or Engineering with 10 years of experience in the relevant field is required).
• Professions license issued by the Ministry of Labor for lecturers.
• Certificate of accreditation of the training program from the Ministry of Higher Education.
• A copy of the Ministry of Health’s approval for the training program in the medical field.

Equipment and tools necessary for work purposes that are used in holding courses

• A letter supporting the application addressed to the Director of the Radiation Protection Department.
• A copy of the registration certificate issued by the Ministry of Trade, Industry and Investment Promotion.
• Certificate of affiliation to the Oman Chamber of Commerce and Industry.
• A list of the names of qualified workers for the purposes of providing these services.
• CVs and experience certificates of qualified workers for the purposes of providing the service.
• Description of the services to be provided and their intended scope.
• Description of equipment and personnel readiness.
• Documents proving your professional competence to perform services.
• A description of the procedures and software used to provide the service.
• Quality management program.
• List of equipment that will be used to provide radiological surveying and maintenance of radiological equipment.
• A list of operational procedures that will be used to provide services.

Obtaining prior approval from the Ministry of Health for its detailed protocol for radiation acceptance testing and quality assurance.

• A letter supporting the application addressed to the Director of the Radiation Protection Department.
• A copy of the registration certificate issued by the Ministry of Trade, Industry and Investment Promotion.
• Certificate of affiliation to the Oman Chamber of Commerce and Industry.
• Copies of the license for vehicles transporting radioactive materials issued by the Civil Defense and Ambulance Authority. This vehicle must be in the name of the organization itself and not rented.
• A list of the names of drivers of vehicles transporting radioactive materials.
• Documents for the participation of all drivers of vehicles transporting radioactive materials in the facility with personal radiation monitoring.
• Any document proving that the company possesses radiation survey meters that accompany the vehicle during transportation.

• A letter supporting the application addressed to the Director of the Radiation Protection Department.
• A copy of the registration certificate issued by the Ministry of Trade, Industry and Investment Promotion.
• Certificate of affiliation to the Oman Chamber of Commerce and Industry.
• A list of the names of employees working in the treatment of radioactive waste.
• Documents for the participation of all workers handling radioactive waste at the facility in personal radiation monitoring (TLD/OSL).
• Any document proving that the company possesses survey meters.
• A program that explains the workflow steps and preventive measures in place during radioactive waste treatment operations.

 A radiation emergency plan in case any radiation emergency occurs during Radioactive material handling.

• A letter supporting the application addressed to the Director of the Radiation Protection Department.
• A copy of the registration certificate issued by the Ministry of Trade, Industry and Investment Promotion.
• Certificate of affiliation to the Oman Chamber of Commerce and Industry.
• A list of the names of employees working in the treatment of radioactive waste.
• Documents for the participation of all workers handling radioactive waste at the facility in personal radiation monitoring (TLD/OSL).
• Any document proving that the company possesses survey meters.
• A program that explains the workflow steps and preventive measures in place during radioactive waste treatment operations.
